Workshop Registration

1.  Complete and print the Workshop Registration Form.

2. Enclose the completed Workshop Registration Form in an envelope, along with your check in the amount of the "Payment Total" shown on the form made payable to the Glass City Quilt Commission, and mail to:

NOTE:  if you include an email address on your form(s), you will receive an email for confirmation of your registration(s). If you would like to receive written notification of the receipt of your workshop registration and payment, please make sure that you have included a self addressed stamped envelope with the form and payment.

Workshop registrations will be accepted on a first come, first served basis until classes are filled.  Workshop registration is not considered complete until payment is received (by July 10, 2017).

Your paid Class or Workshop fee includes a free 1-day pass to the Quilt Show.

2017 Registration

Both the Quilt Entry Form and the Workshop Registration Form that are required to enter a quilted item in the Kaleidoscope of Quilts 2017 show, or to register for a workshop being presented in conjunction with the show, are newly created interactive PDF online forms.  If you encounter any problems using these forms, please scroll down to the "Additional details..." to get more detailed directions for using each form.  This section is located immediately below the links to the two PDF forms.  If you scroll further down the page, you will find the "General Comments..." section that addresses issues such as the possible need to update your free Adobe Reader software, the loss of functionality of these forms with any Apple device, and a method to improved the functionality of these forms for users of Google's Chrome browser.

QUILT ENTRY FORM

The Quilt Entry Form consists of three basic parts that are completed by you, the user:

CONTACT INFORMATION
The blank fields in the first four lines of the form will contain your contact information when completed.

QUILTED ITEM ENTRY DETAILS
Starting with the "Title of Quilt Entry" field in line 5 and continuing down to the "Brief Comments" field will describe the details about your entry.  Mutually exclusive radio buttons are used to select the "Category" of your entry, whether it is an "Entry for Judging," or a "Display Entry."  Simply move your mouse pointer over the desired button and click to select it; clicking on the same button again will de-select it.  Radio buttons are also used to select the appropriate "Technique" and "Quilting Method" to determine the combined category in which your entry will be assigned.  Please do not fail to complete the "Quilted by" and the "Brief Comments" fields, and in the case of the "Children's/Teen's Quilt" category, the "Age of the Maker" field.

You will also need to select the appropriate button for the "Return Method" field that is located to the right of the "For Official Use Only" box.

SIGNATURE & DATE
These two blank fields cannot be completed online, but are instead to be completed by hand on the printed copy of the form that will be mailed in with the Quilt & Workshop Registration Form and your payment.

Upon completion of the online form, you should use your internet browser's print function to print 2 copies of your completed form.  You will need to sign and date one copy to mail in with your Quilt & Workshop Registration Form.  We suggest that you retain the second copy for your records.

If you are only entering one item in the quilt show, you can exit the online Quilt Entry Form by simply using your browser's back key to return you to this Registration page.  In doing so, the online form will close and all data that you entered will be purged.

However, if you will be entering more than one item in the quilt show, you can save some time and avoid re-entering your contact information.    Skip over the contact portion of the form and then starting with the "Title of Quilt Entry" field, modify each field as necessary to describe your next entry, including clicking on different radio buttons as needed.  When you have finished entering the information for this item, repeat the process for printing the modified online form.  This routine can be repeated until a separate Quilt Entry Form has been completed for each of your multiple entries.  When you are done, use your browser's back key to return you to this Registration page.

Quilt Entry Registration

1.  Complete and print the Quilt Entry FormA separate Quilt Entry Form is required for each submission.

2.  If you would like to take a class, complete and print the Workshop Registration Form.  

3.  Enclose your Quilt Entry Form(s) and/or your Quilt & Workshop Registration Form in an envelope, along with your check in the amount of the payment total of all forms (one check may be submitted for the total of all forms added together) made payable to the Glass City Quilt Commission, and mail to:

WORKSHOP REGISTRATION FORM

The Workshop Registration Form consists of four parts, three of which are completed by you, the user, in full functionality mode:

CONTACT INFORMATION
The blank fields in the first four lines of the form will contain your contact information when completed.

QUILT REGISTRATION & PAYMENT
This section of the form indicates the number of quilted items items that you are entering in the show and serves as a payment form.  In full functionality mode, the "Quilt(s) Entry Fee" will be automatically calculated based upon the "Number of Entries Being Registered for Judging" that you enter.  Otherwise, the user will need to manually calculate and enter the "Quilt(s) Entry Fee."  There is no entry fee for Display Entries.  

WORKSHOP REGISTRATION & PAYMENT
This section of the form utilizes check boxes for you to select which workshop(s) that you would like to attend.  To select a specific workshop, simply move your mouse pointer over the appropriate check box and click it.  In full functionality mode, the fee for each workshop selected will automatically be entered in the blank fee field to the right of the workshop listing.  Otherwise, the user will need to complete the blank fee field for each workshop that is selected. 

PAYMENT SUMMARY
In full functionality mode, this section of the form will be automatically calculated and will not require any user input.  Otherwise, the user will need to calculate and enter the "Workshop Fee SubTotal," enter the "Quilt(s) Entry Fee SubTotal," and calculate the "Payment Total."

Upon completion of the online form, you should use your internet browser's print function to print 2 copies of your completed form.  You need to mail in one copy, along with your Quilt Entry Form(s), if any, and your payment in the amount of the "Payment Total."  We suggest that you retain the second copy for your records.

If you are satisfied with your printed results, you can exit the online Quilt & Workshop Registration Form.  Depending upon your browser, this can be done simply by closing the online PDF, or by using your browser's back key to return you to this Registration page.  In doing so, in either case, the online form will close and all data that you entered will be purged.
